Autoimmune pancreatitis: instrumental diagnosis.

چکیده
Autoimmune pancreatitis (AIP) is an unusual type of chronic pancreatitis having an underlying autoimmunity mechanism. Lymphoplasmacytic infiltration and fibrosis on histology, and elevated IgG levels or detected autoantibodies on laboratory data support the concept of AIP. Periductal lymphoplasmacytic infiltration and fibrosis, its preferential occurrence in the pancreatic head, and venulitis characterize autoimmune pancreatitis [1]. AIP should be distinguished from alcoholic chronic pancreatitis because steroid therapy for the former type is effective, morphologic changes are reversible, and pancreatic function can return to normal levels [2]. Difficulties can also arise in differentiating AIP from pancreatic carcinoma, as AIP predominantly affects elderly men, who frequently present with jaundice but without the features of acute pancreatitis, while there can be elevation of serum tumor markers. Other than clinical findings, there are also imaging findings which can be confusing: stenosis of the bile duct, segmental pancreatic enlargement or narrowing of the main pancreatic duct, and angiographic abnormalities which can cause confusion in the differential diagnosis between AIP and pancreatic carcinoma [3]. In fact, in many cases, patients with the focal form of AIP undergo surgery with a clinical and radiological diagnosis of pancreatic carcinoma [4, 5]. In order to obtain a correct diagnosis it is necessary to investigate all available information.

Over the past two decades, the quality of diagnostic imaging in the pancreas has been improved by technical and instrumental progress made in ultrasonography (US), computed tomography (CT) and magnetic resonance imaging (MRI). The examination using ultrasound is inexpensive, non-invasive, and can be rapidly performed. It is often the test of choice in the evaluation of suspected pancreatic lesi...
متن کاملAutoimmune Pancreatitis: A Succinct Overview
Autoimmune pancreatitis is a rare type of chronic pancreatitis with characteristic clinical, radiologic, and histopathologic findings. Diagnosis of autoimmune pancreatitis is often challenging due to its low incidence and nonspecific clinical and radiologic findings. Patients with autoimmune pancreatitis and pancreatic cancer share similar clinical presentations, including obstructive jaundice,...

CONTEXT Autoimmune pancreatitis and sclerosing cholangitis associated with autoimmune pancreatitis respond well to steroid therapy. Some criteria used for the differential diagnosis of autoimmune pancreatitis and sclerosing cholangitis with autoimmune pancreatitis include the response to a steroid trial. CASE REPORT A 68-year-old woman was diagnosed as having type 3 sclerosing cholangitis wit...
متن کامل[Focal autoimmune pancreatitis versus pancreatic cancer: value of steroid treatment in the diagnosis].
BACKGROUND The differential diagnosis of pancreatic cancer and focal forms of autoimmune pancreatitis is complicated since serological tests, IgG4 and CA 19-9 have a low sensibility and specificity. CT scan and magnetic resonance imaging provide clear differentiation in the majority, but not in all cases. Endosonography is the most precise diagnostic procedure and allows to obtain samples for c...
